The information below is intended to provide answers to frequently asked questions and help guide use of the available referral tools.

Referring Patients to Integrative Endodontics

We’ve created online referral tools to make referring patients in need simple and streamlined. We have two options: one form for practices that create their own referral slips, and one for those who do not.

To refer a patient, complete and submit the form with the patient’s information. For patients with multiple teeth that need attention, please be sure to describe the issues or concerns for each tooth.

You’ll receive a confirmation message and email when the form is submitted successfully. Feel free to contact us at [email protected] or (424) 341-3028 to follow up as needed.

Using the Referral Portal

We use CareStack to manage and share patient records and refer patients out. To access referrals and reports, you have to visit the referral portal. We sent you an email which contains your username and password.

Referral Invite

Use the username and password to log into the referral portal: https://endosocal.referrals.carestack.com.

When patient records are shared or patients are referred out, their reports will be available for you to view and download from the portal. First, you’ll receive an email notification.

Referral Notification

When you receive a notification, click the link and log into the portal. There, you’ll see a list of all patients whose information has been shared with your practice. Click “Download” in the Letter column to view and download the report.
